The registration fee for participants includes all educational materials, daily continental breakfast, and refreshments. Deadline for receipt of registration form and fee is October 13, 2017. If we do not receive your registration prior to this date, we cannot guarantee sufficient course materials. A list of participants will not be provided at the conference.

Cancellation requests must be made in writing via email, fax or post marked mail by October 13, 2017 to receive a partial refund. A cancellation fee of $50.00 will be retained on all cancellations before October 13, 2017. Please enclose your original (or copied) confirmation slip with your notice of cancellation. Note: No refunds will be made after October 13, 2017or after the conference begins.
Emory University School of Medicine and Ochsner Health System reserve the right to cancel this conference for any reason at any time. Full refunds of registration fees would be granted to registrants in the event of conference cancellation. The liability of Emory University School of Medicine and Ochsner Health System is limited to the course fee. Emory University School of Medicine and Ochsner Health System will not be responsible for any losses incurred by registrants, including but not limited to airfare, cancellation or hotel deposits.
